Efectivitat del tractament fisioterapèutic del dolor en infants prematurs a la unitat de cures intensives neonatals: protocol d'assaig clínic aleatoritzat

Abstract

Introducció: Els nounats prematurs moderats són els que naixen abans terme, entre 32 i 34 setmanes i 6 dies de gestació. Aquests nens s’ingressen a la Unitat de Cures Intensives Neonatals (UCIN), en la qual reben tot tipus d’intervencions i procediments que poden ser dolorosos. Així doncs, estaran exposats des del primer moment al dolor, el que pot provocar complicacions en el seu futur desenvolupament. Objectius: L’objectiu principal és d’estudiar l’efectivitat de la intervenció de fisioteràpia a la UCIN per millorar el dolor dels infants prematurs nascuts entre les 32 i 34 setmanes + 6 dies de gestació. Metodologia: En aquest estudi es realitzarà un assaig clínic aleatoritzat i controlat, únic sec, amb dos grups, un control i un experimental, de 12 participants cada un. El tractament durarà 3 setmanes, a partir de 35 setmanes post-menstruals fins a 37 setmanes i 6 dies quan el nounat sortirà de la UCIN, i l’estudi 1 any i mig. La població diana d’aquesta intervenció seran els nounats prematurs nascuts entre 32 i 34 setmanes i 6 dies, i ingressats al Centre Hospitalier Régional Universitaire de Montpellier. Per poder formar part d’aquesta investigació els participants hauran de complir els criteris d’inclusió i exclusió. Es recolliran dades quatre vegades durant el tractament, a l’inici i al final, i en el període de seguiment, als 2 i 4 mesos d’edat post-terme. S’utilitzaran les escales següents: PIPP, NISS, NBAS, i TIMP. Llavors s’analitzaran les dades amb el programa SPSS. Limitacions: Les possibles limitacions que podran aparèixer serà el cost de l’estudi, ja que s’hauran de buscar subvencions per finançar l’estudi. També estarem limitats per les escales existents per valorar les variables, les possibles complicacions de salut dels nounats participants en l’estudi i finalment la manca implicació dels pares en el tractament. Introduction: moderate premature newborn are those born before term, between 32 and 34 weeks and 6 days of gestation. These babies are admitted to the Neonatal Intensive Care Unite (NICU), where they receive all kinds of intervention and procedures that can be painful. They will be exposed to pain from the first moment, which can cause complications in their future development. Objective: the main objective is to study the effectiveness of the physiotherapy intervention in the NICU to improve the pain of premature newborns born between 32 and 34 weeks + 6 days of gestation. Methodology: in this study, a randomized and controlled clinical trial will be carried out, with two groups, a control and an experimental one, of 12 participants each. The treatment will last 3 weeks, from 35 weeks post-menstrual age until 37 weeks and 6 days when it will leave the NICU, and the study will last 1 year and a half. The target population of this intervention will be premature newborns between 32 and 34 weeks and 6 day, and admitted to the Centre Hospitalier Régional Universitaire de Montpellier. In order to be part of this research, participants must comply with the inclusion and exclusion criteria. Data will be collected four times, during the treatment, at the beginning and at the end, and in the follow-up period, at 2 and 4 months post-term. The following scales will be used: PIPP, NISS, NBAS, i TIMP. Then the data will be analyzed with the SPSS program. Limitations: The limitations that may appear in this study, will be the cost of it, since subsidies will have to be sought to finance the study. We will also be limited by the scales to assess the different variables, the possible complication health of the newborns and finally the lack of involvement in the treatment from the parents.
