Objective: The objective of our research is to know how the identity of sexual orientation and non-normative gender are constructed in the context of a severe mental disorder of a psychotic type and what role does alcohol consumption play in this triad.
Method: Single case study with a descriptive phenomenological perspective that analyses the gender history of our informant through the lens of intersectionality.
Results: Both gender identity and desire orientation develop, in our informant, anxious and depressive symptoms with recurrent thoughts of death and a frustrated suicide attempt linked to living in heterosexist contexts, internal processes of internalised homophobia (HI) and (nom complet) TI and the consequences of belonging to a social minority. In relation to the consumption of toxics, this appears as a modulating element of all the discomfort resulting from sexual impairment. Schizophrenia disorder develops in a context of consumption and high levels of discomfort produced by HI and TI.
Conclusions: There are fissures between the categories that separate gender and desire orientation in the discourse of the informant. The history of gender cannot be explained as an independent entity from the history of psychological disorder and vice versa. Alcohol appears, on the one hand, as an anesthetizer of the discomfort caused by sexual harm and, on the other hand, as a cause of episodes of violence in the context of positive symptomatology. Having a positive reference, such as a public person undergoing a similar situation, can reduce the discomfort caused by minority stress and prevent HI and TI processes. Sexual impairment and the stigma about mental health could generate interference limiting the social integration of the informant, apart from the residual symptomatology inherent to the disorder.^^^^
