L'acompanyament del malalt crònic des de l'àmbit de les cures pal·liatives

Abstract

El benestar psicològic i la pau interna, espiritual, juguen un paper decisiu per als subjectes que es troben en una situació de malaltia avançada, presentant un pronòstic limitant per a la vida. Aquest pronòstic cursa amb dificultats de tipus psicoemocional, a més de grans limitacions de tipus físic. Aquestes dues vessants, psicològica i física, es retroalimenten i cal que siguin abordades a través d’un treball interdisciplinari. Així, l’atenció al pacient al final de la vida requereix de diversos contextos d’intervenció, incloent l’atenció domiciliària, Centres d’Atenció Primària (CAP) i Centres Hospitalaris, i d’equips de suport.

En aquesta investigació, s’ha volgut estudiar quina percepció té la població general sobre la mort i la tasca que és realitza des de les unitats de Cures Pal·liatives (CP) per, posteriorment, comparar-les amb la realitat viscuda per professionals de la Psicologia en aquestes àrees. Per poder dur a terme la investigació, s’han recopilat dades de dos col·lectius diferents seguint una metodologia d’investigació mixta: el primer grup d’anàlisi ha estat la població general, d’on s’han accedit a les dades de 131 subjectes majors d’edat a través d’enquestes (metodologia quantitativa) i, el segon col·lectiu estudiat han estat els professionals de la Psicologia, entrevistant a un total de cinc participants (metodologia qualitativa).

Tanmateix, s’ha pogut conèixer quina percepció presenten tots dos col·lectius i els resultats subratllen que, des de la població general, hi ha una existència clara de rebuig a la mort i a tot allò que hi està relacionat, un tabú històric i social, destapant un desconeixement en quant al rol de les CP en el procés de mort. D’altra banda, les respostes emeses pel col·lectiu de població general i les emeses pel col·lectiu de professionals, entreveu diferències entre les percepcions d’ambdós col·lectius en relació a l’objecte d’estudi, ja que la població general només té en compte determinats contextos d’intervenció, malalties i aspectes físics que venen influenciats per la manera en com s’entén la mort.

A partir d’aquest treball, doncs, cal que les futures investigacions vagin encaminades a crear programes de divulgació del rol de les CP en la població general per tal d’acostar posicions entre la població general i els professionals. També seria necessari realitzar programes d’educació a subjectes d’edats primerenques, perquè tinguessin un coneixement més real i no estigmatitzat ni negant una realitat envers la mort i el procés de mort, que facilitaria la tasca de cuidar i atendre a persones properes a aquesta etapa final de vida.

Psychological well-being and internal, spiritual peace play a decisive role for subjects who are in a situation of advanced disease, who have a limiting prognosis. This prognosis runs with psycho-emotional difficulties including feelings of severity and difficulty in life, as well as great physical limitations. These two currents, psychological and physical, feedback, and need to be addressed through interdisciplinary work. Thus, end-of-life care requires different intervention contexts and support teams; home care, primary care centers, and hospital centers. In this research, we wanted to study what perception the general population has about death and the task performed from palliative care (CP) and, later, compare them with the reality lived by professionals of Psychology in this area. In order to carry out the research, data have been collected from two different groups following a mixed research methodology: the first group of analyses has been the general population, where data from 131 adult subjects have been accessed through surveys (quantitative methodology) and, the second group studied have been psychology professionals, interviewing a total of five participants. It has been possible to know what perception both groups present and the results underline that, from the general population, there is a clear existence of contempt for death and for everything that is related, uncovering a lack of knowledge regarding the role of CPs in the death process. On the other hand, the responses generated by the general population group and the responses generated by professionals show differences between the perceptions of both groups in relation to the CPs and the death process, because only certain intervention contexts, diseases and physical aspects that are influenced by the way in which death is understood are considered. From this research work, future lines of research aimed at creating programs for the dissemination of the role of the CP among the general population with the aim of bringing closer the perception of the general population and the professionals of Psychology, also about the work they do. Another possible line of research is to design educational programs focused on the child and adolescent population in order to facilitate a real knowledge, not stigmatized or deny a reality about death and the process of dying, facilitating the task of caring for and caring for people in these near-death stages of life.
