Avaluació de l'efectivitat de la introducció de la teràpia assistida amb gossos al tractament de fisioteràpia per a pacients amb paràlisi cerebral espàstica

Abstract

L’objectiu d’aquest estudi és avaluar els efectes de la introducció de la teràpia assistida amb gossos en el tractament de nens amb paràlisi cerebral espàstica. Es tracta d’un assaig clínic aleatoritzat, longitudinal i de caràcter prospectiu, amb una durada de 18 mesos. Es seleccionaran nens i nenes amb edats de 6 a 10 anys amb un nivell 2 o inferior a l’escala d’Ashworth modificada i un nivell 3 o inferior en l’escala GMFCS-E&R, que quedaran dividits aleatòriament en dos grups. El grup control realitzarà tractament de fisioteràpia convencional seguint el concepte Bobath i l’Enfocament Orientat en la Tasca. El grup d’intervenció realitzarà tractament de fisioteràpia convencional i teràpia assistida amb gossos. Els dos grups faran, a més a més, tractament de la motricitat fina amb un terapeuta ocupacional. Per tal d’objectivar els canvis, es recolliran les dades 4 vegades, avaluant-ne el grau d’espasticitat, la motricitat gruixuda, l’habilitat manual, la coordinació i el control postural i l’assoliment dels objectius de teràpia. Limitacions: la grandària de la mostra queda limitada, la possibilitat de produir-se rebuig cap al gos, un cost econòmic superior al tractament convencional i que els hospitals i ambulatoris no donessin el seu consentiment per a captar participants. Finalment, la falta de validesa científica podria dificultar la captació de subjectes.

The aim of this study is to evaluate the effects of the introduction of dog-assisted therapy in the treatment of children with spastic cerebral palsy. It is a randomized controlled clinical trial, longitudinal and prospective, with a duration of 18 months. Children between the ages of 6 and 10, a level of 2 or below in the modified Ashworth scale and level 3 or below in the GMFCS E&R will be selected and divided randomly into two groups. The control group will perform conventional physiotherapy treatment following the Bobath concept and the Task Oriented Approach. The intervention group will perform conventional physiotherapy and dog-assisted therapy. Both groups will also be treated in fine motor skills by an occupational therapist. To objectify changes, data will be collected four times during the study, assessing spasticity, gross motor function, manual ability, coordination and postural control and the achievement of therapy goals. Limitations: the number of participants is limited, there is an increased economic cost compared to conventional treatment, there is a possibility of rejection towards the dog and hospitals and clinics not giving their consent to attract participants. Finally, the lack of scientific validity could affect the recruitment of subjects.
